Report risks, blockers and mistakes early. Urgent concerns must be raised immediately, without waiting for a solution, ranked alternatives or complete proof.
Use measurements, logs, documented limits and engineering judgement to assess constraints. State uncertainty and agree a time limit for investigation rather than demanding mathematical proof for everyday decisions.
Reporting does not make someone the sole recovery owner. Managers agree ownership, supply people and resources, and reprioritise other work. Keep status and changes visible instead of hiding failures.
The CEO and every manager must welcome good-faith concerns, including criticism of leadership. Thank the reporter even when the suggested solution is weak, prohibit retaliation and share the recovery burden.
